Assistive Technology Survey for Caregivers of Older Adults and Individuals with Disabilites

Thank you for sharing your thoughts. We want to make it easier for people with disabilities, older adults, and their families to get assistive technology. This survey will ask questions about how people use Assistive Technology (AT) and any problems they have with it. AT means “tools or devices that help people with disabilities with daily life.” This can include simple items like a long-handled shoehorn or a walker, or tech items like automated lights, a video magnifier, or an electric-assist bike. There is a video below that shows examples of assistive technology at home, school, and work.

This survey is designed for caregivers of people with disabilities, and for caregivers of older adults who need tools to support their daily activities.
