Assistive Technology Survey for Older Adults and Individuals with Disabilites

Introduction

Thank you for sharing your thoughts. We want to make it easier for people with disabilities, older adults, and their families to get assistive technology. This survey will ask questions about how people use Assistive Technology (AT) and any problems they have with it. AT means “tools or devices that help people with disabilities with daily life.” This can include simple items like a long-handled shoehorn or a walker, or tech items like automated lights, a video magnifier, or an electric-assist bike. There is a video below that shows examples of assistive technology at home, school, and work.

This survey is for people with disabilities and for older adults (55+) who use AT devices to help with daily tasks.

If you are a caregiver for a person with disabilities or an older adult (and you are not part of these groups yourself), please fill out our caregiver-specific survey instead: https://www.surveymonkey.com/r/PFN798S
Taking part in this survey is your choice. You can skip any question you do not want to answer. The survey takes about 10 minutes. Your answers are private and will only be shared in a group summary.
