Learning Objectives:
1) Understand the importance of a successful transition program for youth with childhood onset disabilities citing evidence-based practice recommendations
2) Learn about innovative tools and strategies to improve efficiency and effectiveness in providing transition information to patients and families in diverse settings
3) Participants will explore essential characteristics of successful transition models and take home specific tools and resources which can be shared with their patients/families, clinic providers and staff
