Confidential GRACE Survey for Orangewood Church and Orangewood Christian School

This confidential survey is a part of an independent investigation related to past allegations of misconduct by representatives of Orangewood Church, PCA and Orangewood Christian School and a review of the responses by the church and/or school to such allegations. The purpose of this survey is to 1) collect any and all relevant information consistent with the scope of this investigation, and 2) assist GRACE in identifying individuals who may have relevant information that we could contact in order to schedule follow-up interviews relevant to this investigation. Please know that the information provided by survey respondents within the survey itself may be used in the final assessment provided to Orangewood Church, PCA and Orangewood Christian School. However, the identity of survey respondents shall be kept strictly confidential and will not be voluntarily disclosed to Orangewood Church, PCA, Orangewood Christian School, or to any third party without the written permission of the individual survey respondent.

Scope of the Investigation:
“GRACE shall investigate allegations of misconduct by lead Pastor Jeff Jakes when he was the youth pastor of ORANGEWOOD, including but not limited to, how ORANGEWOOD responded to such allegations. GRACE shall also investigate any and all known allegations of sexual abuse upon minors perpetrated by ORANGEWOOD staff or volunteers, including but not limited to, whether ORANGEWOOD had knowledge of such allegations and how it responded.”  (The term “ORANGEWOOD” above refers both to Orangewood Church, PCA and Orangewood Christian School.)

As it relates to this survey, the following definitions apply:

A survivor is any individual (regardless of age or gender) who is a victim of sexual misconduct.

Misconduct is defined as any verbal, nonverbal and/or physical acts of an immoral, indecent, or sexual nature that are 1) unwelcome or 2) performed without consent or 3) committed by one in a position of authority upon a subordinate.

A child is a person 17 years of age or younger.

An adult is a person 18 years of age or older. 

“Without consent” means that consent is not freely given or obtained, and is accomplished through force, intimidation, violence, manipulation, coercion, threat, deception, or misuse of authority or power.

Any person who is mentally or physically incapacitated is incapable of providing consent.

Sexual abuse of a minor (a person 17 years of age or younger) is deemed to be “without consent.” The minor is considered unable to consent due to developmental immaturity and an inability to understand sexual behavior. An offender may perform acts involving sexual abuse against the minor, or the minor may be told, forced, or in any other way, the offender may cause the minor to engage in sexual behavior with the adult. This also includes nude or sexually suggestive or explicit photographic images of a child which are produced, possessed or distributed by any person.
