Thank you for your interest in this study! We are conducting this research project because we want to understand
more about the connections between microbiome, stress response and eating behaviors.
All study participants must meet certain eligibility criteria to make sure that they can participate safely and
provide meaningful data for this project.
First, please complete the following questions and provide your contact information and preferences. Some of
these questions are personal in nature; however, everything disclosed is confidential and will only be shared with
research staff directly involved with this study and those who regulate or sponsor this study. Completion of this
survey implies consent for us to keep the information that you provide.
If you meet eligibility criteria, you will be contacted to provide further information. There is no compensation for
completing this brief survey. However, participants may be compensated up to $850 for full study completion,
which includes 2 study visits and at home tasks over a 2 week period. Participants will complete a 3 hour study
visit of medical screening, interviews and questionnaires, a 3-4 hour study visit with a stress test in which blood
will be drawn, and a 7-day at home period answering questionnaires via smart phone and daily dietary interviews
via phone. This study will involve blood draws, one urine sample and one stool sample.
This survey will take approximately 10 minutes.
