Thank you for your interest in this study! We are conducting this research project because we want to understand more about the relationship between the gut microbiota, the reward value of alcohol and experiences related to alcohol following bariatric surgery.
All study participants must meet certain eligibility criteria to make sure that they can participate safely and provide meaningful data for this project.

Some of these questions are personal in nature; however, everything disclosed is confidential and will only be shared with research staff directly involved with this study and those who regulate or sponsor this study. Completion of this survey implies consent for us to keep the information that you provide.
If you appear to meet eligibility criteria based on this screening, you will be contacted within the next few weeks to provide further information and to schedule a study visit. There is no compensation for completing this brief survey. However, participants who are eligible and decide to participate in the study will be compensated up to $1000. Participation in the study will involve 7 study visits over a 2-year period. Prior to surgery, participants will complete a baseline study visit of medical screening, blood draws, stool sample, Alcohol Purchase Task and other questionnaires, a 10-day at home period wearing an alcohol monitor on their wrist and answering questionnaires via cell phone. Variations of these visits will happen at 6, 12 and 24 months after surgery.
