Thank you for your interest in this study! We are conducting this research project because we are interested in the relationship between alcohol use and experiences related to alcohol following bariatric surgery.
All study participants must meet certain eligibility criteria to make sure that they can participate safely and provide meaningful data for this project.

Some of these questions are personal in nature; however, everything disclosed is confidential and will only be shared with research staff directly involved with this study and those who regulate or sponsor this study. Completion of this survey implies consent for us to keep the information that you provide.
If you meet eligibility criteria, you will be contacted to provide further information. There is no compensation for completing this brief survey. However, participants may be compensated up to $500 for full study completion, which includes 4 study visits over a 1 year period. At 2 time points (one year apart), participants will complete a 4-5 hour study visit of medical screening, a blood draw and cheek swab, psychological interviews and questionnaires, and cognitive testing, and a 14-day at home period wearing an alcohol monitoring device on the ankle and answering questionnaires via smart phone. After the at-home period participants will complete a 15-30 minute study visit to have the monitoring device removed. This study involves stool sample, cheek swab and blood sample collection.
This survey will take approximately 10 minutes.
