Introduction to Survey

Out-of-School Time STEM Survey

Oregon Afterschool for Kids and the Afterschool Alliance are teaming up to gather data on out-of-school-time programs in Oregon that offer science, technology, engineering, and mathematics (STEM) opportunities to school and college-aged children. The goal of this survey is to capture, for the first time, a comprehensive picture of the informal STEM learning opportunities in Oregon. General areas of data collection include: General Program Information, Youth Participation, Program Design and Delivery, Funding and Costs, Staff and Professional Development, Partnerships, and Evaluation.

This survey will identify where out-of-school-time STEM opportunities exist in Oregon and where there are gaps to be filled, as well as document the needs of existing programs. The data from the survey will be compiled into a report and used to communicate with elected officials to promote the need for legislative support and increased funding for out-of-school-time STEM programs.

Your participation in this survey will help support out-of-school STEM programs across the state!

1. Please answer all questions as completely as possible. The better the data we collect, the better our report to lawmakers will be.

2. If you run multiple sites for the same program, please answer with all of the sites in mind. There will be a question asking you to list the addresses of all your program sites; please fill this information out completely. The addresses are our checkpoint to make sure we are not collecting data on the same site twice! Additionally, we will map the data from the survey, and use that map to identify which geographic areas in Oregon have gaps in STEM opportunities for students. We need address information for all programs and all sites for that map to be as accurate as possible. If you have a large number of sites, please feel free to contact us (contact information below) by phone or email, and we would be happy to take site address information in whatever way is easiest for you.

3. This survey allows you to save your responses and resume answering at a later point. To do this, re-open the survey when you are ready to resume by using the original link, click next, and your previous answers will already be filled in. You can click through to the point where you previously stopped, and then resume answering questions. This will only work if you are using the same computer and the same internet browser as you were previously, AND if you haven't cleared your cookies. *

4. Press "Next" to begin the survey. The survey will take about 20 minutes to complete.

For information on what programs should take the survey, or if you have other questions, please consult our FAQs, or contact Katie Lakey at or 971-282-5325

Thank you for your participation!
