
Hospice Palliative Care Ontario Outstanding Philanthropist Award |
The Hospice Palliative Care Ontario Outstanding Philanthropist Award recognizes individuals or families; or corporations, foundations, or civic service organizations which have demonstrated exceptional generosity and outstanding civic and philanthropic leadership dedicated to advancing hospice palliative care in their community or throughout Ontario.
Eligibility
Eligibility
- The candidate should have been involved in supporting the field of hospice palliative care for at least five (5) years in Ontario.
- Professional fundraisers, individuals that fundraise from others, and captive foundations (e.g. a hospital foundation, a hospice foundation) are not eligible.
Award Criteria
- You must state the total amount donated by the individual, family, or organization.
- Evidence of direct gift(s) and a description of the impact of the gift(s). Direct means a personal donation by the nominee as an individual/family or the total amount if an organization (corporation, foundation, civic, or service organization).
- Evidence of encouragement and motivation to others to lead by example on philanthropy.
- Evidence of philanthropic support of innovative approaches to solving problems.
This award is intended to recognize outstanding philanthropy having significant impact on hospice palliative care. The total amount donated by an individual, family or organization is a substantial factor considered by the adjudication team. The sum of multiple donations by an individual, family, or organization to one or more hospice palliative care organization will be considered.
