Introduction and Purpose
Hello,
My name is Andy Draper, I am the Research Coordinator for the Center for Community Studies at JCC in Watertown.
This survey is not to ask for money or donations. We are conducting a needs assessment on behalf of the Lewis County Office for the Aging.
As a valuable member of our community, the Lewis County Office for the Aging is interested in hearing what you have to say about your concerns about living in Lewis County. Their mission is to help make Lewis County a great place for older adults. They would like your help in determining any concerns you have which may not enable you to live independently in your community.
The hope is that you will assist us in determining any areas of concerns in our community to support our residents as they age, by taking a few minutes to complete this brief survey.
IMPORTANT - To complete this survey you must live in Lewis County and be at least 18 years of age.
