HealthPathways Checklist

Northern Territory HealthPathways will be a free web-based portal that is available for use by clinicians at the point of care, to access evidence-based, locally accessible patient pathways.

Northern Territory PHN is in the initial stages of building HealthPathways, and we are gathering information to help clinicians refer to your service, the more information you are able to provide will help improve the quality of referral to your service.
