NPS SEPAC Needs Assessment

Thank you for completing our annual SEPAC Needs Assessment Survey. All responses are completely confidential. If you have any questions or would like to discuss any part of the survey, please contact northamptonsepac@gmail.com.


This survey is 18 questions and should take approximately 5 minutes to complete.  Please respond to these first questions so we can get a better understanding of our SEPAC membership.

12.Lastly, it is SEPAC’s role to conduct a needs assessment to assist in PAC's regulatory responsibility to participate in the planning, development, and evaluation of the school district's special education program. The following questions relate to that needs assessment related to your child’s IEP/504 plan process and implementation.

IEP/504 plan evaluations conducted by school personnel accurately reflect my child's needs.
