Steps for Submission:
  1. Complete this application completely.
  2. Submit PI-CME Participant Disclosure Forms for the Project Team Leader and ALL participants to the CME Coordinator.
Upon receipt of this application and ALL Participant Disclosure Forms, the CME Committee may accredit this project as a PI-CME activity. 

For an approved PI-CME activity:  upon successful completion of each project stage as outlined below, participants may be awarded AMA PRA Category 1 credit™ as described within this application.
The Performance Improvement Activity (PI-CME)

Natividad's CME Program awards CME credit for approved team-based Performance Improvement activities that includes two or more unique disciplines (e.g., physician/nurse, nurse/pharmacist, etc.) and the inclusion of other team members, as appropriate.  The Performance Improvement project - quality and safety initiatives, designed to achieve care performance improvements, must adhere to an evidence-based and metrics-driven framework. 

Up to 20 CME credits per project for physicians may be awarded.  Other team members receive participation certificates.  Natividad's CME credit may be eligible for CE/EMS credit from Natividad's Nursing Education.

  • team member roles and accountabilities;
  • criteria/methodology for earning and reporting participant credit.
PI-CME Three Stage process*:

    Stage A.   assessment of current practice (Plan);
    Stage B.   application of intervention(s) to practice (Do);
 
   Stage C.   reassessment and reflection (Study/Act).

*At least one physician must be part of the team with additional disciplines involved as appropriate in order to offer CME credit.
