2018 Community Health Needs Survey |
Dear Community Member,
New London Hospital wants to learn about the health of our community and we want to hear from YOU.
“Community” can mean different things to different people. For this survey, your community can be as big as the Ragged-Kearsarge-Sunapee-Newport geographic area or a group of people with whom you associate. Please answer our questions by thinking about the area or people you see as “your community.”
New London Hospital is conducting this survey along with Dartmouth-Hitchcock, Alice Peck Day Memorial Hospital, Valley Regional Healthcare, Mt. Ascutney Hospital and Health Center and Visiting Nurse and Hospice for VT and NH. Survey results will help us shape our community health programs.
Please take about 10 minutes to give us your thoughts and opinions. The survey is completely anonymous. You will not be asked for your name or contact information.
