Client Feedback Survey

The Hastings Prince Edward Ontario Health Team (HPEOHT) is looking to evaluate the COVID, Cold and Flu Care Clinics that were held in Tweed, Belleville, and Picton this fall and winter. We want to know whether these types of clinics are useful during cold and flu season and if we should offer these clinics in the future. If you attended one of these clinics during this time period and we would like to hear from you. If you are a guardian or caregiver for someone who attended one of these clinics please answer the questions on their behalf. The survey should only take you about 5 minutes to complete. This information is confidential and individual responses will only be seen by the evaluator with the HPEOHT to compile aggregate information.
