Client Satisfaction Survey Phase 2
About this survey
Thank you for taking the time to complete the second part of our client survey! The Parent Advisory Committee in collaboration with Safe & Sound staff have developed this survey to learn more about your experience with our services and how we can improve it. We also took your feedback on last year's survey and incorporated it in this one as much possible. This survey will be longer than the last one since we are asking more questions about each program.
Your responses will be kept private and confidential so please answer as honestly as you can.
This survey may take about 20 minutes and as a token of appreciation for your time, we will give you an electronic gift card of $20 after completion of the survey. Please follow instructions at the end on how to get it.
