This survey will be used to confirm who is attending the May 5-6, 2016 Operations meeting at Library of Congress. It is intended for BIBCO, CONSER, NACO and SACO operations representatives, standing committee chairs, PoCo representatives, and others associated with the PCC who may be attending. We will use the data for planning the meeting venue and other activities. The survey data will allow us to print name tags and the attendee list.

We plan to make reservations for a dinner on Thursday May 5, 2016. More information on restaurant choice(s) will be made available when the details are finalized. If you plan to attend the dinner, please answer "Yes" to question 5.
