CTIA Super Mobility 2015 MobileCON Speaker Release Form

* I hereby grant CTIA–The Wireless Association® (“CTIA”), with respect to the photograph, audio and/or video recording that is taken of me during the CTIA Super Mobility 2015 event on September 9-11, 2015 at the Sands Convention Center in Las Vegas, Nevada (the “Conference”), the absolute and irrevocable right and permission:

1. To copyright the video recording in CTIA’s name;

2. To use, re-use, publish and republish the same in whole or in part, individually or in conjunction with other photographs, film, video or images, in any medium and for any non-commercial purpose whatsoever, including, but not limited to, illustration, promotion, advertising and trade, and

3. To use my name, my employer’s name, logo, trademark and products in connection therewith if CTIA so chooses.

Further, I agree to provide to CTIA, by the time of my presentation/session/talk, a final copy of any written or audiovisual materials that I present at the Conference. I grant to CTIA the absolute and irrevocable right and permission to use, re-use, publish and republish any such materials for non-commercial purposes, including to the extent such materials are incorporated within any photograph, audio and/or video recordings referenced herein.

I hereby release and discharge CTIA and their respective affiliated organizations, their directors, officers, employees and agents from any and all claims and demands arising out of or in connection with the use of such video, including, but not limited to any claims for defamation, invasion of privacy, or infringement, to the extent such use is consistent with the rights I am granting CTIA.

I am of the legal age of 18 or older and have read the foregoing and fully understand the contents thereof.
