This online Membership Application is for Club WISE, MODE and/or WISE Diner. You can apply for one or all three programs. Age limits apply.
This application is secure and it will take you approximately 5 minutes to complete.
To join Club WISE, there is a membership fee of $20 ($10 for Santa Monica residents from City funding support). There is no fee to become a member of MODE and/or WISE Diner (for eligible Santa Monica residents only). Please note that the driver may arrive 15 minutes prior to or 15 minutes after the reserved pick-up or return time.
Required Documents:
A valid picture ID is required for ALL three WISE & Healthy Aging membership programs and proof of residency is required. For Santa Monica residents, acceptable proofs of residency are utility bills (gas, electric, water, trash) or a copy of your lease agreement. Telephone or cable bills are not accepted. All statements must a have current address. Please do not submit bank or financial information.
Paper application: Applications can also be submitted in person or mailed to WISE & Healthy Aging at 1527 4th Street, Santa Monica, California 90401 - Attention: Member Services. You may also email it to: memberservices@wiseandhealthyaging.org.
