The Many Faces of Community Health Conference Planning Committee is in the process of preparing for yet another annual conference. This will be the 14th year of the Many Faces of Community Health Conference, which will take place October 31 – November 1, 2019 at the Earle Brown Heritage Center in Brooklyn Center, MN.

This year we would like to pay special recognition to individuals that have been at the forefront of leading innovations in healthcare, improving care delivery models, and other community health initiatives that promote health equity, prevent and manage chronic diseases, and ensure access for all.

We are pleased to announce that the nomination period for these three annual awards is now open! We are inviting you to submit nominations for one or all of the award categories. Select an award on the following page to learn more about eligibility and selection criteria, and to submit a nomination. Nominations are due Friday, August 30th for the three awards:

       ·  Outstanding Community Health Center Employee

       ·  QI/Innovative Project Award

       ·  Safety Net Champion

To be considered, nominations must be received by close of business on Friday, August 30th, 2019.

Award winners will be recognized during the 2019 Many Faces of Community Health Conference. Winners will receive a complimentary registration to attend the conference on the day they are presented their award.
