MSNA Hero Site Award Nomination Form

This award showcases EXCELLENCE in Teamwork, Creativity and Customer Service. Going the “Extra Mile” is a part of the team’s goals to bring their best service to the forefront.

Entry Guidelines

Any team of two or more may apply for this award.
Each team must be member of SNA/MSNA.
Each team member must be employed by a school nutrition program.
Application must originate and be submitted by the kitchen team.
Nominations must be submitted online by Friday, April 7, 2023.

Recognition

Certificate, Letter to Principal, Director and an Invitation to President’s Reception at Annual Conference.
Recognized for School Lunch Hero day (May 5), being featured on the MSNA website, social media, and newsletter.
