MSNA Hero Award Nomination Form

The MSNA Hero award recognizes an individual who stands out as a Hero to their peers, students and staff.

Entry Guidelines

Individuals must be nominated by any staff member within the school district. Self-nominations are not accepted.
The nominated member must be a school nutrition employee. Directors are not eligible for this award.
Each individual must be a SNA/MSNA member.
Multiple candidates may be nominated from each district.
Please note that question 5 asks for two statements from others in the district regarding this nomination. Please provide the name and email address of those providing the statements.
Nominations must be submitted online by Friday, April 7, 2023.

Recognition

Certificate, Letter to Principal, Director and an Invitation to President’s Reception at Annual Conference.
Recognized for School Lunch Hero day (May 5), being featured on the MSNA website, social media, and newsletter.
