Capital One Supporter of the Year - Nomination Form

Please use the below form to nominate a supporter to be recognised at this year's Football League Awards. Nominations are invited for fans of any current Football League club.

The judges are looking for someone they felt has gone above and beyond the call of duty and added that extra special bit of devotion to their club. Each of the shortlisted finalists will be the Supporter of the Year in their division, with one crowned the overall Capital One Supporter of the Year. Please note, all entries will be checked and validated with the nominee's club.

The closing date for nominations is 12pm on Wednesday, 9th March 2016.
