The Michigan Department of Health and Human Services will be providing free qualitative fit testing apparatus kits and Michigan Occupational Safety and Health Administration (MIOSHA) training resources to ensure respiratory protection standards are met throughout the State. These fit testing kits are for use with respirators and include a hood, collar, 2 nebulizers, bitter tasting sensitivity solution, bitter tasting fit test solution and laminated user instructions.
Please fill out this form to request fit kits and training resources for your facility. This form should be filled out by the facility Infection Preventionist (or other facility employee), who will be the point of contact for the respirator test kit training.
Note: Shipments of Fit Testing Kits are submitted on a monthly basis.
1. After submission of this survey, your information will be provided to our vendor who will then ship 2 fit testing kits to the address provided.
2. You will also receive an email with training resources for use in establishing a respiratory protection program.
If further assistance is required, consultative assistance from MIOSHA can be requested through a link that will be provided with the training resources.
