Emergency Management (Residential Aged Care Facilities No 41 (COVID-19) Direction which came into effect at 10:57am on Friday 20 August 2021 requires all South Australian Residential Aged Care Facilities (RACF) to have a
COVIDSAfeCheckIn QR code from 10 September 2021 to assist with contract tracing in the event of an outbreak.


To assist RACF, this quick survey has been developed to collect the information required for the Department of Premier and Cabinet to generate and issue a QR code to each RACF site.

It is mandatory for all South Australian RACF's to complete all questions in this survey by 5pm, Friday 27 August 2021. The survey should be completed for each site with a unique RACF ID.

QR codes will be sent to each RACF prior to the implementation date of 10 September 2021.
