Help Us Improve Primary Health Care in New Westminster and the Tri-Cities
We're looking to better understand the experiences of patients who have visited the Emergency Department in New Westminster or the Tri-Cities.
This short survey will support local Family Doctors, Primary Care Providers, and Hospital teams in improving transitions in care - from the hospital back to your main source of care (such as a family doctor or walk-in clinic).
