Draft Patient Values - Inviting Your Input

The Excellent Care for All Act, legislated by Ontario's Ministry of Health and Long-term Care, puts patients first. By June 2011 all hospitals will have a Patient Declaration of Values developed with input from the public.

We would like to hear from you. You are invited to read a draft Patient Declaration of Values document, created by the Patient and Family Centred Care Steering Committee, with patient and family advisors. After reading the document, we invite you to answer the following questions.
