Aging Task Force: Senior Needs Assessment (SHORT) v.2

 

 

 

This survey is about community services available to Gaston County senior citizens age 60 and older, and their caregivers. Caregivers can be relatives, friends, or neighbors providing any type of assistance to a senior living in Gaston County. The survey will take about 20 minutes to complete. The answers you give will be kept private. Your information will be used by the Gaston County Aging Task Force to plan for future aging services in Gaston County.

 

 

 

 

If you are a caregiver that is under 60 years of age for a Gaston county senior, please answer the survey questions based on your senior's experiences and opinions. Answer for them, not yourself. Caregivers who complete the survey do not need to be a decision-maker for the seniors for whom they provide care.
