ATTENTION LUMMI NATION ELDERS (AGE 55+)

This application is for both the 2026 Community Basic Needs Distribution and the Elder Distribution.

If you will be 55 or older by December 31, 2026, please complete this application.

You do not need to complete a separate Elder Distribution application.

Eligible elders will automatically be directed to the additional Elder Distribution certification as part of this form.

If you will not turn 18 by December 1, 2026, a parent or legal guardian must complete the Minors Basic Needs Distribution application on your behalf.
