Gambling Attitudes and Behavior Survey

This survey is being conducted for the Lakeshore Regional Entity to support planning to prevent and address problem gambling in Allegan, Kent, Lake, Mason, Muskegon, Oceana, and Ottawa counties.

 The survey collects information about gambling behaviors and attitudes. The information you provide will be kept private. No one will know how you answer. Please answer the questions based on what you really do or think.

Completing the survey is voluntary. If you are not comfortable answering a question, just leave it blank.
