1. Consumer & Carer Consultation

The AMHC service model is set by the Department of Health, however, we are keen to consult on the aspects we are able to influence. Specifically, we are  interested in how we can localise the model to meet the needs of our ACT community, along with wanting to consult on the outcomes we are aiming to achieve from a consumer point of view. 

To aid the consultation process, we are seeking your input on a series of questions you will find below.  Responses will be de-identified and collated before any reporting.  The purpose of this survey is to inform our decision making and planning for the new centre.  Your input is invaluable and we encourage you to provide as much detail as possible in the free text sections to help us understand your perspective.
