Incident Report
All incidents including near misses and omissions, whether minor or significant must be reported accurately and quickly to enable Living My Way to investigate and take appropriate action to prevent similar occurrences in the future. Your cooperation in providing detailed information is invaluable and together we can create a safer environment for everyone.
As a NDIS Registered provider, Living My Way is required to notify the NDIS Commission of ‘reportable incidents’ that have or have alleged to have occurred in connection with the provision of supports and services within 24 hours of becoming aware of the incident or allegation. These incidents include:
- The death of a person with disability.
- Serious injury of a person with disability.
- Abuse or neglect of a person with disability.
- Unlawful sexual or physical contact with, or assault of, a person with disability.
- Sexual misconduct committed against, or in the presence of, a person with disability, including grooming of the person.
- The use of a restrictive practice in relation to a person with disability that is unauthorised use or not in accordance with a behaviour support plan.
Living My Way and you are also required to make a report to Department of Communities and Justice (DCJ) when there is a concern about the safety, welfare and wellbeing of a child (0 - 15 years) and they are at risk of significant harm. This includes physical abuse, neglect, sexual abuse, psychological harm, danger to self or others, concern about the carer and/or carer relinquishing care.
