The New Hampshire Department of Health and Human Services' Health Professions Data Center collects key practice and capacity data from all licensed APRNs in the state during license renewal. This survey is designed to collect general information regarding your current provider status and the locations at which you routinely clinically practice.

The data is a key resource in statewide health care workforce assessment and health care access planning. It informs federal shortage designation, policy, emergency preparedness, workforce pipeline programs, and recruitment and retention initiatives including the National Health Service Corps and the NH State Loan Repayment Program.

The data collected is confidential and protected; it will only be used for the aforementioned purposes.

Only APRNs due to renew an active status NH license expiring in 2026 should complete the survey.

The survey is a requirement of license renewal and is complete when all responses are submitted by selecting "Done" at the end of the survey.
