You are invited to participate in a research study conducted by Edwin Purdy, a student in the Leadership Department at Converse University in Spartanburg, SC. You must be at least 18 years of age to participate in this study. Please read this form and ask any questions you may have before agreeing to be in the study.
PURPOSE OF STUDY: The purpose of this study is to research visual arts teachers’ perspectives on culturally responsive teaching. Data collected from this study will be used in a doctoral dissertation and possibly published by the researcher.
PROCEDURES: If you agree to take part in this study, you will be asked to complete a survey that will take about 5-10 minutes online using Survey Monkey.
VOLUNTARY PARTICIPATION: Participation in this study is voluntary. You may skip any questions that you do not want to answer or stop participating at any time. There is no penalty for not participating.
RISKS and BENEFITS There are no anticipated risks or discomforts to your participation in this study other than those encountered in daily life and personal reflection on teaching practices, but if any of the questions are upsetting, please seek counseling through your school or district.
Although you may not directly benefit from this research, by participating in this study, the analyzed data will provide research on the field of visual arts and culturally responsive teaching and the opportunity to improve practice over time.
CONFIDENTIALITY: No participant names will be used for this study. My supervising professors and I will be the only people with access to survey data. Hard copies will be kept in a locked filing cabinet, and electronic copies on a password-protected external hard drive that will also be in a locked filing cabinet when not being used to compile information.
COMPENSATION: You will not be paid for participating in this survey.

CONSENT LETTER
I am at least eighteen years of age. I have read this form, and the research study has been explained to me. I am fully aware of the nature and extent of my participation in this research project and the possible risks as outlined above. I understand that I may withdraw my participation on this project at any time without prejudice or penalty of any kind. I hereby agree to participate in this research project.
