We Need Your Help!
The Ko-Kwel Wellness Centers (KWC) in Coos Bay and Eugene continue to provide services and increase access to health care. We are asking for feedback on your experience in 2022 when using the KWC services. The information is used to develop improvements to better serve Tribal members, patients, and clients at both locations.
- Medical Clinics and Pharmacy Services
- Purchase and Referred Care (PRC)
- Community Services and Family Support programs
- Behavioral Health services
- Oral Health Care services
To help make this survey task a little zestier, we will be offering a weekly giveaways for participation. Winners will be selected by random drawing each Friday at 3 p.m. The prize drawings will be done each week until the survey closes on Friday, May 5, 2023. If you want to participate in the giveaways, please include your name and contact information in the survey.
NOTE: Your responses to this survey are not tied to your name. Your responses are collected for reports and review without identifying you personally. The only KWC staff who will see your contact information will be the Quality Improvement Coordinator Anna Chavez, who analyzes the response data, and conducts the weekly prize drawing. If you choose not to include contact information, anonymous submissions are accepted but will not go into the prize pool.
Everyone who completes this survey will also be entered into the grand prize drawing for a $100 Amazon gift card!
