Please note that this application is for attending the conference or a pre- or post-training certification course in Nashville, TN. Attendees must be able to provide their own hotel accommodations, flight, transportation, and meals. There will be no virtual options to attend. 

Scholarship Application
In service to the field, NABITA welcomes the submission of scholarship applications for its upcoming 2023 Case Management Summit with Pre- and Post-Training Certification Courses. These scholarships aim to assist our colleagues in their professional development and commitment to their chosen profession. Scholarships will be awarded to applicants based upon written responses to the questions below. A limited amount of scholarships will be awarded, and submission of this application does not mean a scholarship will be awarded. Please complete the application below in full, providing detailed and complete answers for each question. Only full, complete applications will be considered.

Application and Submission Instructions
Applications must be completed in full. Partial applications will not be considered.

Only one application per person can be received. Applicants should choose either to apply for a conference scholarship or pre- or post-training certification course scholarship. Applicants may not apply for both. 

Applications for multiple scholarships will not be considered. Applications cannot be co-authored. Each individual must submit their own application.

The receipt of a scholarship award in previous years does not preclude applicants from submission for or receipt of a scholarship for this conference.

Scholarships that are awarded cover registration fees only.

Scholarships will be awarded directly to applicants. Should award recipient be unable to attend our event for any reason, scholarship dollars are not transferrable to another member of the awarded institution/organization. Scholarship dollars are not to be credited or carried over to other events. Should recipient be unable to attend, the scholarship may be awarded to another applicant. Additionally, a high level of consideration will been given to your application in regards to financial need. If you or your organization has made payment for your attendance prior to receipt of award notification, registration dollars will not be refunded and your scholarship award with be forfeit.
