2023 Health Fair Vendor Registration Form

The Canal Winchester Chamber is hosting their 7th Annual Running Scared 5K and Health Fair on October 28th from 9AM - 12PM. Please fill out this form in it's entirety to register your company for a Health Fair booth.

An email will be sent out a few days prior to the event with a location and parking details. Call/Text the Chamber at 614-837-1556 on the day of the event if you have any questions. Thank you for your interest!
