NOMINATION FORM

Nominations are now being accepted for the  2017 Summit Education Awards.  Award recipients will be announced and recognized during the Sheboygan County Workforce Development Summit on September 26, 2017 at Blue Harbor Resort & Spa.
 
Please consider nominating someone for the following awards:
  • Sheboygan County Educator of the Year Summit Award
  • Sheboygan County Business Friend of Education Summit Award

Deadline for submission is Wednesday, July 26, 2017 at 5:00 p.m. 

* 1. Nominator Information

* 2. The Sheboygan County Educator of the Year Summit Award honors a teacher or school counselor who has positively impacted students through innovative education programs, mentoring or who goes above and beyond the call of duty to prepare students for success in life after graduation.

Eligibility
  • Must be a teacher or counselor who is employed by a school district within Sheboygan County
  • May represent primary or secondary education
  • Previous Golden Apple Award Recipients are not eligible for nomination; however past nominees may be nominated again
Criteria
  • Successfully demonstrates having a positive impact on Sheboygan County students as a result of their programs, actions or initiatives
  • The impact should be viewed as long term, enhancing and easily recognizable
  • Demonstrates a commitment to self-growth and professional development through continuing education

* 3. The Sheboygan County Business Friend of Education Summit Award honors an individual or business who has positively impacted students through successful partnerships with education, innovative education programs, mentoring or initiatives that encourage students toward employ-ability excellence.

Eligibility
  • Must be an individual employed in Sheboygan County or a business located in Sheboygan County
  • The actions, programs or initiatives must benefit Sheboygan County students
Criteria
  • Successfully demonstrates having a positive impact on students as a result of their programs, actions or initiatives
  • The impact should be viewed as long term, enhancing and easily recognizable
  • Demonstrates a successful and sustaining partnership with Sheboygan County schools and/or educators
