2015 Jay County Hospital Convenient Care Survey

We recently conducted a series of interviews and focus groups to identify community healthcare needs. During that process we clearly heard the need for a new care option during times when family physicians' offices are closed.

To address this need, we are considering an affordable, convenient new option through our emergency room and would value your feedback. The following survey should take three to five minutes to complete and will provide important insight as we continue our efforts to improve the health of our communities.
