This survey is being conducted by the National Council of Urban Indian Health and supported by the San Manuel Band of Mission Indians (CA). It aims to capture a detailed picture of needs among urban Native American and Alaska Natives (AI/AN or “urban Indian” are used interchangeably) living with disabilities in California, Oregon, and Washington.

We aim to identify some of the challenges that urban Indians living with disabilities face. In particular, we would like to identify barriers in accessing services and any gaps in current service provision. 

The survey findings may be used to influence policy decisions as well as developing and improving services.

If needed, a caretaker, relative, friend, etc. may assist you in completing this survey--including entering your responses.
