I grant permission for my child to participate in ‘The 9th MGM Junior Lion Dance Summer Class 2023’. I have full knowledge of the content and the nature of this program. I have read and agreed to be bound by all terms and conditions. I expressly agree and give my consent to photo shooting and filming and the use of the material. I declare that the participant is in good health and physical condition without any injuries or medical conditions making him / her unfit to participate in sports.

Terms & Conditions
1. Applications are accepted from May 25 to June 4, 2023.
2. Participants must be between the ages of 5 and 8 on June 30, 2023.
3. Parent / guardian / emergency contact may apply for the child. A valid ID copy of the child must be provided.
4. Body check results from certified doctors must be provided to prove that the participants are capable of participating in the summer class.
5. Should applications exceed the intended capacity (80), a randomized computer draw will select successful applicants. Successful applicants will be informed via email on June 7, 2023.
6. In case of insufficient participants, MGM reserves the right to cancel or merge classes.
7. Both student and parent should understand and be responsible for any and all personal risks and responsibilities throughout the event. They do not have the right to demand MGM take any and/or all responsibilities for any form of compensation for any accidents which may occur during or are caused by the event.
8. Participants must follow coaches' instructions and the venue's rules. MGM reserves the rights to terminate the participant's enrollment in case of noncompliance.
9. MGM reserves the right to photograph and videotape in classes for use in future promotions.
10. MGM pledges to keep all personal data confidential; used solely for the purpose of the administration and processing of this application.
11. MGM management reserves the right to final decisions in case of any dispute or rearrangement of the event.
12. In case of discrepancies between the Chinese and English versions of the Terms and Conditions, the Chinese version shall prevail.
