Thank you for participating in the Joint Fire Science Program Knowledge Exchange Evaluation! The participation of both new and continuing respondents (those who have responded to versions of this survey in the past) is needed to help improve the performance of your regional Fire Exchange, and to better understand your fire science information experiences and needs. It is estimated that it will take 10 minutes or less of your time to complete this new shortened survey.

If you choose to participate, you will be able to complete the questionnaire by following the simple instructions provided. There are a few things you need to know before starting:

- The evaluation questionnaire asks about experiences and opinions related to fire management and fire science research in your region. In addition, the survey contains some demographic items regarding your gender, ethnicity, occupation and general geographic region.

- Once you begin the questionnaire, you will not be able to return to complete it at a later time. Please complete it before logging out.

- Although the evaluation does not contain questions that are sensitive in nature, you may wish to complete the questionnaire in a secure location so that others will not be able to view your responses. The evaluation questionnaire does not ask for any personal identifying information, and thus is anonymous. Your responses will be kept confidential and your participation in this study is completely voluntary. You can withdraw from participation at any point and choose not to answer any question(s).

This evaluation is being administered by the University of Nevada, Reno, as an external entity not affiliated with the Joint Fire Science Program.
