If you find detailed surveys difficult, a brief version for people with such disabilities may be accessed here.
JFK Partners will soon compete for federal funds for our program in Intellectual and Developmental Disabilities, including autism. These funds support specialized training of people who work with Coloradans with disabilities and their families, as well as disability research, community education, and collaboration with community partners. Your input will help us shape our five-year plan for serving Colorado’s disability community.
To shorten our questions, “I/DD” is substituted for Intellectual and/or Developmental Delays and Disabilities and autism. You will notice that we repeat the I/DD topics in Questions 5-8 to learn how each area should be addressed in our program. Questions about this survey can be sent to Dina.Johnson@cuanschutz.edu.
Confidentiality: Surveys collected by JFK Partners are voluntary. Confidential information will not be shared with others, and answers will be used for the purposes for which they were collected.
