Illinois ACEs Response Collaborative 2019 Survey

Thank you for participating in the Illinois ACEs Response Collaborative (the Collaborative) 2019 Survey. The purpose of this survey is to gather information for our strategic planning process. All responses will be kept confidential. Individual responses will be aggregated and reported in a summary document that will identify basic themes. The survey should take just 10 minutes to complete, and it will be open during the month of January. Your willingness to share your experiences with and thoughts on the work of the Collaborative and on the field of ACEs, trauma, and resilience is much appreciated.
