Introduction

Thank-you for participating in this survey - the purpose of which is to gain a better understanding of Canadian General Surgeons’ views and practices regarding intercostobrachial nerve preservation during axillary lymph node dissection for breast cancer surgery.

This survey has a total of 19 questions that will take approximately 5 minutes for you to complete. For each question, please respond by checking the one box which best reflects your opinion or practice, unless otherwise indicated.

 All survey responses will be kept anonymous and your personal information and contact will not be collected.

 All survey questions are optional for you to answer and you may withdraw your participation in the survey at any point in time.  However, partial or full completion of the survey implies consent to use your responses for the study.

Due to the contents and questions in the survey, the Hamilton Integrated Research Ethics Board has confirmed that this survey does not need to be reviewed by the ethics board.
