FY 23- FY 24 Service Area Needs Assessment for Center for Life Resources

Center for Life Resources is conducting a survey to better understand the needs of people with mental health and/or substance use disorders and intellectual and/or developmental disabilities.   Responses are anonymous and will be used to plan services, reduce barriers to services, and coordinate services with other area providers for fiscal years 2023 and 2024.    If a question makes you feel uncomfortable, you may skip it.  Please answer as many questions as possible.  Please complete the survey by June 30, 2022.   if you have knowledge or experience with the mental health services substance abuse disorders treatment, and/or services for individual with intellectual disabilities and/or developmental delays in the following counties:  Brown, Coleman, Comanche, Eastland, McCulloch, Mills and San Saba.
