The COVID-19 pandemic has had a profound effect on all people in Canada, including our Heart & Stroke community. We hope to continue to understand its impacts on you and your health, how you have managed and adapted, and your needs going forward.

Heart & Stroke values your input to help guide the work we do and ensure it is relevant in meeting the needs of people living with heart conditions, stroke, and vascular cognitive impairment. Results from our first COVID-19 survey have allowed us to raise the needs of people with lived experience with media, government and within our virtual offerings.

The survey will take approximately 15 minutes to complete.  

This survey includes a component for people with lived experience (PWLE) of heart conditions, stroke and vascular cognitive impairment and a separate component for caregivers of PWLE (i.e. family or informal/unpaid regular in-person support person).

Note: If you are both a person with lived experience and also a caregiver and wish to complete both surveys, you are welcome to do so. Please complete one version of this survey entirely from one point of view, and then relaunch the survey and complete another version. Your responses will be treated separately and will not be linked together.

Please note:

Anyone living with ongoing chronic disease, such as heart disease or stroke, who experiences a change or worsening in symptoms should seek timely medical treatment by contacting their healthcare team or going to their local hospital.
