Phase 2 - Community Consultation

The Hamilton Niagara Haldimand Brant Local Health Integration Network (HNHB LHIN) is developing its Integrated Health Service Plan (IHSP) 2019-2022. The IHSP guides health system transformation, health service delivery and funding decisions over a three-year period. We want to hear what’s important to you!

While we’re sharing some of the themes and priorities we’ve heard so far during the first phase of our community engagement, we know there are more voices to be heard. In addition to this survey, we’ll also be holding online webinars throughout July and August to encourage further dialogue and solicit real-time feedback.
