Dear Valued Customer:
     The goal of the Phillips Companies has and always will be to serve its customers with the highest in quality products and services.  In order for us to continue to meet that goal, we ask that you take a few minutes to complete the following customer satisfaction survey.  Your responses to this survey will be kept confidential and anonymous and will be processed by Strategic Leadership Associates, our consulting firm. They will report to us only on the overall trends and preferences of the survey as a whole and will not disclose individual information about your company.
     In exchange for your thoughts and ideas, we will send you a $5 Speedway gift card and our appreciation.
