Project Background
Funded by the New York State Department of Health (NYSDOH), PCAP is UWNYC's comprehensive community health program that strengthens the capacity of community- and faith-based organizations to improve access to preventative healthcare and reduce health inequities in underserved neighborhoods across New York City.

Many New Yorkers continue to face significant barriers to care, including limited access to primary care, unmet social needs, lack of health insurance, language and cultural barriers, transportation challenges, and distrust of healthcare systems. As a result, communities with the greatest disparities experience disproportionately high rates of chronic diseases—including diabetes, hypertension, cardiovascular disease, and obesity—as well as poor maternal health outcomes and unmet behavioral health needs.

PCAP addresses these challenges through an integrated, community-centered model that combines preventative health services, maternal health, behavioral health support, Food is Medicine programming, and care coordination. By investing in trusted community organizations, PCAP builds neighborhood-based infrastructure that connects residents to healthcare, social services, and other essential resources to improve long-term health outcomes.
Award Details
United Way of New York City (UWNYC) is pleased to launch the application for the 2026–2027 Preventative Care Access Program (PCAP) Connect2Care (C2C) Initiative. Through this funding opportunity, UWNYC is seeking 30-35 community- and faith-based organizations serving priority neighborhoods across New York City's five boroughs to deliver community-based preventative care programming between October 2026 and March 2027.

Selected partners will receive $62,000 to expand access to preventative healthcare and social services in communities experiencing the greatest health disparities. The initiative aims to increase access to care, identify and address chronic disease risk factors, connect residents to life-saving health and social services, reduce premature mortality, and strengthen the capacity of community-based organizations to serve as trusted hubs for integrated public health programming.
The Connect2Care Model
Connect2Care (C2C) is the cornerstone of the Preventive Care Access Program. Through funding, workforce development, technical assistance, care coordination infrastructure, community engagement, and clinical partnerships, C2C equips trusted community organizations to serve as neighborhood hubs for PCAP's preventive health and navigation services.

The model is built around five core program components:
  1. Workforce Development and Organizational Capacity Building: Community partners receive funding to support a full-time Health Advocate, who leads community outreach, coordinates programming, and provides care coordination and service navigation, as well as a part-time Project Lead, who oversees program implementation, administration, and reporting. Together, they are responsible for delivering program activities and strengthening the organization's public health capacity. Partners also receive comprehensive training, technical assistance, and professional development in care coordination, health education, behavioral health, leadership, management, and other topics tailored to their organizational needs. Additional resources include access to UWNYC's online training library and ongoing capacity-building support.
  2. Clinical Service Delivery: Partners collaborate with a healthcare provider contracted with UWNYC to bring free preventive health services directly into their communities through monthly community-based events. Services may include health screenings, vaccinations, primary care connections, and other preventive clinical services that reduce barriers to care.
  3. Community Health Education: Partners deliver culturally responsive, community-informed “Lunch & Learn” health education events focused on chronic disease prevention, nutrition, maternal health, behavioral health, and overall wellness to empower residents with the knowledge and resources needed to make informed decisions about their health.
  4. Care Coordination and Referral Infrastructure: Health Advocates conduct Social Determinants of Health (SDOH) screenings and provide individualized care navigation to connect community members with healthcare providers and social services. All partners receive access to UWNYC's care coordination platform which supports Health Advocates to conduct screenings, create referrals to clinical & social support services, and track participant outcomes.
  5. Community Outreach and Engagement: Partners conduct proactive outreach to engage residents and connect them to preventive health services and resources. Through collaboration with neighborhood institutions—including schools, NYCHA developments, faith communities, and other community organizations—partners increase awareness of available services and expand participation in Connect2Care programming.
Partner Expectations
Selected partners will be expected to:
  • Staff the program: Hire or designate a full-time Health Advocate and part-time Project Lead within thirty (30) days of program launch.
  • Deliver health programming:
    • Host one (1) health screening event per month in coordination with a UWNYC-contracted clinical partner, serving a minimum of 30 participants per event.
    • Host at least two (2) health education workshops per month delivered by the Health Advocate or UWNYC-contracted facilitators, serving 20–30 participants per event.
  • Provide care coordination: Conduct at least ten (10) social determinants of health (SDOH) screenings per month and provide care coordination and closed-loop referral navigation for individuals with identified needs.
  • Conduct community outreach: Recruit and engage community members to participate in program activities and health events.
  • Engage NYCHA: Collaborate with at least one NYCHA development to host a health screening event during the contract period (UWNYC will provide connections to NYCHA Resident Services, if needed)
  • Participate in workforce and organizational development:
    • Attend monthly workforce development trainings focused on health education, care coordination, and related topics.
    • Participate in at least one organizational capacity-building training during the contract period.
Eligibility Criteria
Organizations should, at minimum, be able to demonstrate that they:
  • Are a 501(c)(3) nonprofit community- or faith-based organization with at least two years of operating history, or operate under a fiscal sponsor (documentation required at the time of application)
  • Are based in, or primarily serve, New York City, with priority given to organizations serving one or more of the following target populations: uninsured or underinsured individuals; low-income individuals and families, including those living at or below 200% of the Federal Poverty Level (FPL); individuals living with chronic disease and/or behavioral health challenges; NYCHA residents; Black and Latinx birthing people; Medicaid-enrolled populations; and immigrant populations.
  • Have experience delivering community-based health programming, such as health screenings, health education, wellness workshops, peer support, counseling, fitness activities, or similar services. Additional experience in food access, maternal health, mental health, and/or climate resiliency is preferred.
  • Have experience conducting community outreach and engagement, including canvassing, flyering, digital outreach, advocacy, or similar strategies to connect community members to services.
  • Have experience providing social support services, including conducting social determinants of health (SDOH) screenings, care coordination, service navigation, or referrals through platforms such as FindHelp, Unite Us, or similar systems.
  • Have organizational capacity to hire or assign required project staff within 30 days of award and have access to facilities capable of hosting events for approximately 35 participants.
  • Maintain an active Board of Directors, appropriate organizational insurance coverage (e.g. disability, workers' compensation and general liability), and current financial compliance (e.g., IRS Form 990, audited financial statements, as applicable).
  • Have the linguistic and cultural capacity to effectively serve the communities they propose to engage.
  • Current or returning UWNYC partners only: Are in good standing with UWNYC, including compliance with reporting, contractual, and fiscal requirements.
Contract Term
It is anticipated that the contract term will run from approximately October 19, 2026 through March 31, 2027.

UWNYC reserves the right to extend or renew the contract based on funding availability, contractor performance and programmatic needs.

Selected organizations will be required to submit additional compliance and contracting documents upon award notification and prior to contract execution.
Budget Criteria
Funds may be used for the following:
  • Salaries and Fringe, including at least one full-time Health Advocate and one part-time Project Lead; salaries shall be prorated for the contract term between October 19, 2026 and March 31, 2027
  • Meals and Refreshments for “Lunch & Learn” health education events
  • Printing and Copying of outreach, health education, and event materials
  • Program Materials and Supplies
  • Event-related Travel Expenses within NYC
  • Space Rental for events
  • Staff Trainings to build skills around health, wellness, facilitation and care coordination (outside of UWNYC trainings)
  • Indirect Costs up to 10%, to cover overhead expenses including space, insurance, and other administrative costs
Any additional expenses beyond this list will require UWNYC approval.
Selection Process
UWNYC will only consider proposals that satisfy the Eligibility and Budget Criteria listed above. Applications will be scored on the following:
  • Demonstrated ability to meet project goals based on organizational program experience, capacity, and past partner performance
  • A realistic budget aligned with proposal description and goals
  • Alignment with UWNYC’s mission and program goals
Preference will be given to organizations that operate in the target communities. Both new applicants and returning partners will be equally considered.
Grant Terms and Reporting Requirements
If awarded funding, grantees must:
  • Submit compliance documents including Form 990, 501(c)3 letter of determination, recently audited financial statement, and general liability, workers compensation, and disability insurance
  • Attend an in-person orientation the week of October 19, 2026 to discuss program goals, requirements, and expectations.
  • Submit a Project Action Plan within 45 days of program launch.
  • Participate in monthly cohort meetings and check-ins with the assigned UWNYC Program Manager.
  • Host at least one (1) site visit by the UWNYC Program Team during the contract period.
  • Submit monthly program reports and invoices in accordance with UWNYC reporting requirements. Failure to comply with reporting will result in grant termination and may lead to non-consideration for further funding.
